We are currently accepting new patients. To schedule an appointment, please call (317) 449-5631. New patients, be sure to fill out these forms and bring them with you to the first visit.

Cancellation Policy
|
24 hours notice is required to cancel or reschedule an appointment. Appointments not cancelled or rescheduled with at least 24 hours notice will be assessed a fee equal to what would have been charged for the estimated service.

Check-In Procedures

If you are a new patient, please arrive 15 minutes early for your appointment and fill out these forms prior to your arrival.

Fees |
Accepted forms of payment are cash, check, and major credit cards. Fees are collected at time of service. A fee schedule is available upon request by telephone. As we operate a fee-for-service model, we do not currently accept or process insurance claims. We provide receipts that include all of the necessary information for patients to seek reimbursement from their insurance company if they choose.

Please leave a message and your doctor will return your call as soon as possible during normal business hours M-F: 8am to 5pm. Please be aware that medication refills will only be called in during normal business hours.
